Acupuncture

Scientific Evidence, Clinical Practice and Christian Discernment

“Among all complementary therapies, acupuncture has generated one of the largest bodies of scientific research. At the same time, it raises important questions concerning physiology, philosophy and worldview. These dimensions require careful distinction if acupuncture is to be evaluated responsibly.”

Chapter Overview

Acupuncture originated within Traditional Chinese Medicine (TCM) more than two thousand years ago and remains one of the most widely practised complementary therapies worldwide.

Traditionally, acupuncture is explained through concepts such as:

  • Qi (vital energy);
  • meridians;
  • yin and yang;
  • balance within the body.

Modern biomedical research, however, has proposed alternative explanations involving:

  • peripheral nerve stimulation;
  • spinal cord modulation;
  • endogenous opioid release;
  • autonomic nervous system regulation;
  • central nervous system plasticity.

This chapter critically examines both traditional explanations and contemporary scientific evidence. Particular attention is given to separating measurable physiological effects from the philosophical framework in which acupuncture historically developed.

Finally, the chapter evaluates acupuncture from a Christian perspective, distinguishing between evidence-based clinical practice and worldview assumptions that may conflict with biblical teaching.

5.1 Introduction

Acupuncture has become increasingly integrated into healthcare systems across the world.

In some countries it is practised primarily by physicians and physiotherapists with additional training, whereas elsewhere it is offered by practitioners trained within Traditional Chinese Medicine.

Its popularity reflects several factors:

  • increasing interest in non-pharmacological pain management;
  • patient preference for complementary approaches;
  • concerns regarding long-term medication use;
  • expanding scientific investigation.

Despite this growth, important questions remain.

Do the traditional concepts of Qi and meridians correspond to measurable anatomical structures?

Can acupuncture produce clinically meaningful effects independent of its traditional explanatory model?

How should healthcare professionals evaluate conflicting interpretations of its mechanism of action?

These questions require careful examination using both scientific methodology and philosophical clarity.

5.2 Historical Development of Acupuncture

Origins in Ancient China

The origins of acupuncture are generally traced to ancient China several centuries before the Common Era.

Early Chinese medical texts describe illness as arising from disturbances in the harmonious circulation of Qi, the vital life force believed to flow throughout the body.

According to this worldview, health depends upon maintaining balance between complementary forces known as:

  • yin;
  • yang.

Disease develops when this balance becomes disturbed or when the flow of Qi is obstructed.

Treatment therefore aims to restore harmony by stimulating specific points located along invisible pathways called meridians.

These concepts became foundational to Traditional Chinese Medicine and continue to influence acupuncture education in many parts of the world.

Classical Medical Texts

One of the most influential works in the history of acupuncture is the Huangdi Neijing (The Yellow Emperor’s Inner Canon).

Compiled over several centuries, this text discusses:

  • diagnosis;
  • meridian theory;
  • acupuncture techniques;
  • philosophical concepts;
  • relationships between humans and nature.

Although enormously important historically, these writings emerged long before the development of modern anatomy, physiology and experimental science.

Consequently, they should be understood within their historical and cultural context rather than interpreted as scientific textbooks in the contemporary sense.

Development Through the Centuries

Acupuncture evolved considerably over time.

Different schools proposed varying numbers of:

  • meridians;
  • acupuncture points;
  • needling techniques;
  • diagnostic methods.

During later centuries acupuncture spread to:

  • Korea;
  • Japan;
  • Vietnam;
  • other regions of Asia.

Each culture adapted aspects of the original Chinese system, producing distinct traditions that continue today.

Introduction to the West

European physicians first encountered acupuncture through missionaries, traders and physicians returning from East Asia during the seventeenth and eighteenth centuries.

Interest remained limited until the twentieth century, when increasing cultural exchange stimulated wider investigation.

During the 1970s acupuncture attracted international attention following reports of its use for postoperative pain and other medical conditions in China.

Since then thousands of clinical studies have examined its potential therapeutic value.

Importantly, the scientific evaluation of acupuncture has increasingly focused on measurable clinical outcomes rather than acceptance of its traditional philosophical explanations.

History Does Not Determine Scientific Validity

Many medical practices possess long histories.

Historical longevity alone, however, neither proves nor disproves clinical effectiveness.

Some ancient treatments have later been validated by modern research, whereas others have been abandoned after scientific investigation demonstrated ineffectiveness or harm.

Consequently, the historical importance of acupuncture should be respected without allowing antiquity itself to determine judgments concerning clinical efficacy.

Evidence Summary 5.1

Acupuncture developed within the philosophical framework of Traditional Chinese Medicine, where health and disease were explained through concepts such as Qi, meridians and the balance of yin and yang. As acupuncture spread internationally, modern biomedical research increasingly investigated its clinical effects using contemporary scientific methods. To evaluate acupuncture responsibly, it is first necessary to understand the conceptual framework in which it originated. The next section examines the central concepts of Traditional Chinese Medicine—including Qi, meridians, yin and yang and the Five Elements—before considering how these ideas relate to modern anatomy, physiology and evidence-based medicine.

5.3 The Traditional Chinese Medical Model

Understanding the Conceptual Framework of Acupuncture

Introduction

Acupuncture did not arise as an isolated treatment technique.

It developed within the comprehensive philosophical and medical system known as Traditional Chinese Medicine (TCM).

This system includes theories concerning:

  • health and disease;
  • diagnosis;
  • physiology;
  • emotions;
  • environmental influences;
  • therapeutic interventions.

Central concepts include:

  • Qi;
  • meridians;
  • yin and yang;
  • the Five Elements.

These concepts form an internally coherent explanatory model but differ fundamentally from the anatomical and physiological models used in contemporary biomedical science.

Understanding these concepts is essential before evaluating either their scientific status or their theological implications.

5.3.1 Qi

The Central Concept of Traditional Chinese Medicine

Within Traditional Chinese Medicine, Qi is regarded as the fundamental life force underlying all physiological activity.

Qi has been described in various ways, including:

  • vital energy;
  • life force;
  • functional activity;
  • dynamic vitality.

According to classical theory:

  • Qi circulates continuously throughout the body.
  • Health depends upon harmonious circulation.
  • Disease results from deficiency, stagnation or imbalance of Qi.
  • Acupuncture restores normal flow by stimulating specific acupuncture points.

Importantly, no universally accepted definition of Qi exists even within Traditional Chinese Medicine.

Different historical schools have interpreted the concept differently.

Modern Scientific Perspective

From the perspective of contemporary biomedical science, Qi presents important challenges.

Modern anatomy has not identified a measurable anatomical structure corresponding to Qi.

Similarly:

  • no universally accepted biochemical marker;
  • no recognised physiological measurement;
  • no reproducible imaging technique;

has demonstrated the existence of Qi as traditionally described.

Consequently, many contemporary researchers regard Qi primarily as a historical explanatory concept rather than an experimentally verified biological entity.

Others interpret Qi metaphorically as representing integrated physiological function rather than a distinct physical substance.

These differing interpretations should be distinguished carefully.

Distinguishing Observation from Explanation

A patient reports reduced pain following acupuncture.

The reduction in pain represents an observable clinical outcome.

The explanation that pain improved because Qi was restored represents one possible interpretation rather than an experimentally established scientific conclusion.

Scientific investigation therefore focuses on measurable physiological mechanisms rather than assuming the correctness of traditional explanatory models.

5.3.2 Meridians

The Pathways of Qi

Traditional Chinese Medicine teaches that Qi circulates through a network of invisible channels called meridians.

Classical descriptions identify twelve principal meridians associated with major organ systems, together with several additional extraordinary vessels.

Along these pathways lie hundreds of acupuncture points.

Needle insertion at these locations is believed to influence Qi flow and thereby restore physiological balance.

Relationship to Human Anatomy

Modern anatomical investigation has not identified physical structures corresponding directly to the classical meridian network.

Unlike:

  • arteries;
  • veins;
  • nerves;
  • lymphatic vessels;

meridians are not recognised anatomical structures within standard human anatomy.

Numerous hypotheses have attempted to relate meridians to:

  • connective tissue planes;
  • fascial networks;
  • peripheral nerves;
  • interstitial fluid pathways.

Although some anatomical correlations have been proposed, no hypothesis has achieved broad scientific consensus.

Consequently, contemporary biomedical explanations of acupuncture generally focus on known neurophysiological mechanisms rather than classical meridian theory.

5.3.3 Yin and Yang

Dynamic Balance

The principles of yin and yang represent one of the oldest philosophical concepts within Chinese thought.

Rather than absolute opposites, yin and yang describe complementary aspects of reality.

Within Traditional Chinese Medicine:

Philosophical Character

Unlike measurable physiological variables such as blood pressure or serum glucose concentration, yin and yang function primarily as philosophical categories.

They organise clinical observations within a traditional conceptual system rather than representing independently measurable biological substances.

For this reason, many historians regard yin–yang theory as a pre-scientific explanatory model reflecting the worldview of ancient Chinese natural philosophy.

Within Traditional Chinese Medicine, disease is interpreted as disruption of this balance.

5.3.4 The Five Elements

Traditional Chinese Medicine also incorporates the doctrine of the Five Elements (or Five Phases).

These are:

  • Wood;
  • Fire;
  • Earth;
  • Metal;
  • Water.

Unlike the chemical elements of modern science, these categories describe patterns of relationship and transformation.

Each element is associated with:

  • organs;
  • emotions;
  • seasons;
  • colours;
  • tastes;
  • climatic influences.

Within TCM these relationships assist diagnosis and treatment planning.

Modern Evaluation

From the standpoint of contemporary biomedical science, the Five Elements should be understood as part of a traditional philosophical classification system rather than experimentally verified physiology.

Modern medicine does not classify organ function according to elemental correspondences.

Nevertheless, understanding this framework is important because it continues to influence diagnosis and treatment in many schools of acupuncture.

Clinical Practice Box 5.3

Understanding the Patient’s Perspective

A patient explains that their acupuncturist diagnosed a “Liver Qi stagnation.”

The physician recognises that this terminology belongs to the diagnostic language of Traditional Chinese Medicine.

Rather than dismissing the patient’s experience, the physician respectfully asks about the symptoms that led to this diagnosis and then translates them into conventional clinical terminology where appropriate. This promotes clear communication while avoiding confusion between traditional concepts and biomedical diagnoses.

5.3.5 Traditional Diagnosis

Diagnosis within Traditional Chinese Medicine differs fundamentally from diagnosis in modern medicine.

Traditional methods commonly include:

  • observation of the tongue;
  • pulse examination at several positions;
  • detailed questioning;
  • assessment of sleep, appetite and emotional state;
  • evaluation of perceived patterns of imbalance.

The purpose is not primarily to identify pathological anatomy or biochemical abnormalities but to classify the patient’s condition according to TCM patterns.

Examples include:

  • Qi deficiency;
  • Blood deficiency;
  • Dampness;
  • Heat;
  • Cold;
  • Yin deficiency;
  • Yang deficiency.

These diagnostic categories guide acupuncture treatment within the traditional system.

Evidence Summary 5.2

Traditional Chinese Medicine provides a comprehensive conceptual framework based on Qi, meridians, yin and yang, the Five Elements and pattern-based diagnosis. These concepts have shaped acupuncture practice for centuries and remain influential in many educational traditions. However, contemporary biomedical science has not identified direct anatomical or physiological equivalents for several of these concepts. Consequently, modern research increasingly distinguishes between the historical explanatory model of Traditional Chinese Medicine and experimentally testable neurophysiological mechanisms that may account for the clinical effects of acupuncture.

Transition to §5.4 – Modern Neurophysiological Explanations of Acupuncture

The next section examines how contemporary neuroscience seeks to explain acupuncture without relying on traditional concepts such as Qi or meridians. Research into peripheral nerves, spinal cord modulation, endogenous opioids, brain imaging and autonomic regulation has produced a growing body of evidence that offers biologically plausible mechanisms for at least some of acupuncture’s observed clinical effects.

5.4 Modern Neurophysiological Explanations of Acupuncture

From Traditional Theory to Experimental Science

Introduction

During the past five decades, acupuncture has become the subject of extensive biomedical research.

Rather than attempting to verify the traditional concepts of Qi or meridians, most contemporary investigators have focused on measurable physiological processes that may explain the observed effects of needle stimulation.

Research has examined changes occurring at multiple levels of the nervous system, including:

  • peripheral sensory nerves;
  • the spinal cord;
  • brainstem nuclei;
  • higher cortical centres;
  • autonomic nervous system regulation;
  • neurochemical signalling.

Although experimental research identifies several biologically plausible responses to needling, no single mechanism explains every reported clinical effect, and mechanistic findings should not be treated as proof of condition-specific clinical benefit.

5.4.1 Peripheral Nerve Stimulation

The insertion of an acupuncture needle stimulates numerous structures within the skin and underlying tissues.

These include:

  • free nerve endings;
  • mechanoreceptors;
  • muscle spindles;
  • connective tissue receptors;
  • small blood vessels.

Mechanical stimulation generates electrical impulses that travel through sensory nerve fibres toward the spinal cord and brain.

Depending on the depth and location of needling, different types of sensory fibres may be activated.

These include:

  • A-beta fibres;
  • A-delta fibres;
  • C fibres.

Each fibre type contributes differently to sensory processing and pain modulation.

Needle rotation may also deform surrounding connective tissue.

Experimental studies suggest this mechanical interaction may influence:

  • fibroblast activity;
  • extracellular matrix organisation;
  • local biochemical signalling.

Whether these changes contribute significantly to clinical outcomes remains under investigation.

Current evidence indicates that these tissue responses probably complement rather than replace neural mechanisms.

Clinical Reflection Box 5.4

Local and Systemic Effects

A patient receives acupuncture for chronic shoulder pain.

Needling produces a local twitch response accompanied by temporary soreness.

Within minutes the patient experiences a reduction in pain extending beyond the immediate needling site.

This observation suggests that acupuncture may involve both local tissue responses and central nervous system modulation.

5.4.2 Spinal Cord Mechanisms

One of the earliest biomedical explanations of acupuncture derives from the Gate Control Theory of Pain, proposed by Melzack and Wall.

According to this model, stimulation of large sensory nerve fibres can inhibit transmission of pain signals within the dorsal horn of the spinal cord.

Although modern pain science has expanded considerably beyond the original gate control model, spinal modulation remains an important component of pain regulation.

Experimental models suggest that needling may influence:

  • inhibitory interneurons;
  • neurotransmitter release;
  • ascending pain pathways.

These effects may reduce transmission of nociceptive signals to higher brain centres.

5.4.3 Endogenous Opioid Release

One of the best-studied mechanisms involves activation of the body’s endogenous opioid system.

Experimental studies, including animal and laboratory research, have reported changes involving substances such as:

  • endorphins;
  • enkephalins;
  • dynorphins.

These naturally occurring peptides participate in the regulation of pain perception.

Several observations support this mechanism:

  • acupuncture-induced analgesia may be reduced by opioid antagonists such as naloxone;
  • animal studies demonstrate altered opioid receptor activity;
  • neurochemical analyses reveal increased endogenous opioid concentrations following needling.

Endogenous-opioid signalling is a plausible contributor to some analgesic responses, but it does not establish that acupuncture produces a clinically important benefit for a particular condition.

5.4.4 Brain Imaging Studies

Modern neuroimaging techniques have provided new insights into acupuncture.

Functional magnetic resonance imaging (fMRI) and positron emission tomography (PET) studies have reported changes in activity in brain regions associated with:

  • pain perception;
  • emotional regulation;
  • attention;
  • autonomic function.

Reported regions include:

  • anterior cingulate cortex;
  • insular cortex;
  • thalamus;
  • somatosensory cortex;
  • periaqueductal grey.

These findings suggest that acupuncture influences distributed neural networks rather than acting exclusively at the site of needle insertion.

Nevertheless, neuroimaging demonstrates correlation rather than proof of therapeutic effectiveness.

Changes in brain activation do not automatically establish clinically meaningful benefit.

5.4.5 Neurotransmitter Modulation

Acupuncture has also been associated with changes in several neurotransmitter systems.

Studies have reported alterations involving:

  • serotonin;
  • noradrenaline;
  • dopamine;
  • gamma-aminobutyric acid (GABA);
  • glutamate.

These neurotransmitters influence:

  • pain modulation;
  • mood;
  • sleep;
  • autonomic regulation.

The precise clinical significance of these biochemical changes remains incompletely understood.

Moreover, different acupuncture techniques may activate distinct neurochemical pathways.

Clinical Practice Box 5.5

Biological Plausibility Versus Clinical Effectiveness

A researcher demonstrates increased endorphin release following acupuncture in healthy volunteers.

While this finding provides a biologically plausible mechanism, it does not by itself establish that acupuncture produces clinically meaningful benefits for patients with chronic pain. Clinical effectiveness must still be demonstrated through well-designed randomised controlled trials that measure outcomes important to patients.

5.4.6 Autonomic Nervous System Regulation

Several studies suggest that acupuncture may influence the autonomic nervous system.

Reported physiological changes include:

  • alterations in heart rate variability;
  • modest changes in blood pressure;
  • effects on gastrointestinal motility;
  • modulation of stress responses.

These observations have stimulated interest in acupuncture for disorders involving autonomic dysregulation.

However, findings remain heterogeneous, and methodological differences between studies limit firm conclusions.

Further high-quality research is required to clarify the magnitude and clinical relevance of these effects.

Evidence Summary 5.3

Modern neuroscience has identified several biologically plausible mechanisms by which acupuncture may influence pain perception and other physiological processes. These include activation of peripheral sensory nerves, modulation of spinal cord transmission, release of endogenous opioids, changes in brain network activity and effects on autonomic regulation. Importantly, these mechanisms can be investigated using established scientific methods and do not require acceptance of traditional concepts such as Qi or meridians. Nevertheless, the demonstration of physiological effects does not automatically establish clinical effectiveness, which must be evaluated through rigorous clinical trials.

Transition to §5.5 – Clinical Evidence for Acupuncture

Although modern neuroscience provides plausible mechanisms for some effects of acupuncture, the key question for clinicians remains whether these physiological changes translate into meaningful improvements for patients. The next section critically reviews evidence from randomised controlled trials, systematic reviews and clinical guidelines across major medical conditions, distinguishing areas where acupuncture has demonstrated benefit from those where evidence remains uncertain or insufficient.

5.5 Clinical Evidence for Acupuncture

Evaluating Effectiveness Through Evidence-Based Medicine

Introduction

The central question for clinicians is not whether acupuncture produces measurable physiological responses, but whether these responses translate into clinically meaningful improvements for patients.

Evidence-based medicine distinguishes between:

  • biological plausibility;
  • physiological effects;
  • statistical significance;
  • clinically meaningful benefit.

Consequently, acupuncture should be evaluated using the same standards applied to every therapeutic intervention.

The strongest evidence comes from:

  • systematic reviews;
  • meta-analyses;
  • large randomised controlled trials;
  • evidence-based clinical guidelines.

5.5.1 Chronic Musculoskeletal Pain

Chronic musculoskeletal pain is among the most extensively studied groups of indications, but evidence and recommendations differ by diagnosis, comparator, outcome, treatment protocol and jurisdiction.

Conditions studied include:

  • chronic low back pain;
  • neck pain;
  • shoulder pain;
  • osteoarthritis;
  • myofascial pain syndromes.

Systematic reviews suggest that acupuncture may provide small or modest improvements in pain or function for some patients; differences from no treatment are generally larger than differences from credible sham procedures.

However, several important observations should be recognised.

Benefits are generally:

  • modest rather than dramatic;
  • greatest in carefully selected patients;
  • often comparable to other conservative therapies;
  • influenced by patient expectations and contextual factors.

For selected chronic-pain presentations, acupuncture may be considered as one component of a comprehensive multidisciplinary plan when consistent with the current condition-specific guideline, practitioner scope and patient preferences; it should not displace active rehabilitation or established care.

Clinical Practice Box 5.6

Chronic Low Back Pain

A 58-year-old woman has experienced chronic low back pain for four years despite exercise therapy and non-opioid analgesics.

Following discussion of the available evidence, acupuncture is offered as an adjunct to her existing rehabilitation programme rather than as a replacement for exercise, education and physical therapy.

After eight sessions she reports moderate improvement in pain and mobility, allowing greater participation in daily activities.

This example illustrates the role of acupuncture as part of integrated pain management.

5.5.2 Headache Disorders

Acupuncture has been investigated extensively for:

  • migraine prevention;
  • tension-type headache.

Several large systematic reviews indicate that acupuncture may reduce headache frequency in selected patients.

Some condition-specific guidelines recognise a limited course of acupuncture for migraine prevention when clinically appropriate. Because recommendations and eligibility criteria differ by jurisdiction, the applicable current guideline should be checked, particularly when:

  • preventive medication is ineffective;
  • medication is poorly tolerated;
  • patients prefer non-pharmacological treatment.

Evidence is generally stronger for prevention than for treatment of acute migraine attacks.

5.5.3 Osteoarthritis

Knee osteoarthritis represents one of the most extensively studied musculoskeletal disorders.

Many trials report:

  • modest pain reduction;

While these concepts remain important for understanding the historical development of acupuncture, contemporary biomedical science has not identified direct anatomical or physiological equivalents for them.
